Health Center

The SPFC Health Center provides primary and some specialty health care services for adults aged 18 and older without private or government insurance.

The SPFC Health Center offers basic health and wellness services by a combination of volunteer and staff health care providers including physicians, nurse practitioners and registered nurses. Patients with diabetes can receive referrals to the Healthy Living Coach and Health Education services.

Who is Eligible?

Now accepting NEW patients!

Our services are intended for individuals who are uninsured. To become a patient, you must be:

  • Must be age 18 or older.
  • Must be uninsured.
  • Income must be at, or less than, 300% of the federal poverty level.

Before You Arrive

  • Please arrive 15 minutes before each of your scheduled appointment times.
  • Bring your ID and a list of any current medications or supplements.

FAQs

Please contact our office to schedule an appointment. Our team will conduct a brief interview to determine your eligibility. We will need to know your current income and income for those in your household.

If you have recently seen another medical provider or were recently discharged from the hospital, you will need to come in to complete the necessary forms so that we may obtain your medical records prior to your visit. If we do not obtain those records, we may not be able to see you.

If you do not meet all the eligibility requirements, there are other resources in our area that may be able to serve you.

Please visit Pinellas Florida Dept. of Heath and Evara Health. If you are still having difficulty finding care, please call our office or you can contact our Client Advocate at 727-821-1200 ext 126 to discuss other resources.

Individuals that are unhoused can check with Bayside Health Clinic.

We do not accept walk-in appointments at this time. Please contact our office and we will schedule an appointment for individuals that qualify. This helps us ensure an appointment with the appropriate medical professional for your condition is arranged.
