General Information
The SPFC Health Center provides primary and some specialty health care services for adults aged 18 and older without private or government insurance.
The SPFC Health Center offers basic health and wellness services by a combination of volunteer and staff health care providers including physicians, nurse practitioners and registered nurses. Patients with diabetes can receive referrals to the Healthy Living Coach and Health Education services.

Who is Eligible?
Our services are intended for individuals who are uninsured. To become a patient, you must be:
- Must be age 18 or older.
- Must be uninsured.
- Income must be at, or less than, 200% of the federal poverty level.
Before You Arrive
Please arrive 15 minutes before each of your scheduled appointment times. Bring your ID and a list of any current medications or supplements.
