Who is Eligible?
Our services are intended for those who lack access to medical care due to lack of dental insurance. To become a patient, you must be:
- Must be age 18 or older.
- Must be uninsured.
- Must meet the low-income status of being at, or less than, 200% of the federal poverty level.

Please bring a photo ID with your current address or photo ID and proof of residency.
